  • Position Objective and Summary:




    • As part of a team, the YVLifeSet Case Specialist provides direct individual and family casework services to facilitate healing through trauma Informed services and Positive Youth Development. Using the YVLifeSet model and UCAN clinical philosophy, YVLifeset Case Specialist will serve youth between the ages of 17-21 years of age who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood.

      The skills to be developed include:



      • Obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills.
      • Obtaining, and maintaining employment, building a support system, and following the law to remain free from legal involvement.
      • In addition, the YVLifeSet Case Specialist will ensure all parenting youth served are provided with supportive resources to pursue education to enhance parenting skills and to prepare youth for self-sufficiency upon aging out of care.


      Educational and Work Qualifications:



      • Bachelor's degree in social services with one-year related experience in counseling and or case management required; Master's degree in social work, psychology or related field preferred.
      • Demonstrated understanding of trauma-informed practice.
      • Working knowledge of community resources to use in serving clients and maintains productive working relationship with these resources
      • Demonstrated understanding of Positive Youth Development.
      • Ability to work flexible and/or varying shifts, including evenings and weekends.
      • Proficiency in operating a computer with Microsoft Office suite of applications.
      • Ability to interpret and accurately apply the regulations of federal and state programs.
      • Respect for cultural and socioeconomic characteristics of clients and coworkers.
      • Ability to manage multiple priorities simultaneously.
      • Excellent writing skills and record keeping practices.
      • Excellent interpersonal skills. Valid Illinois driver's license and insurance, access to a car, and the ability to transport children in accordance with IDCFS regulations.
