Under general direction of Care Management leadership, the Clinical Social Worker provides social work services consisting of discharge planning, continuing care services and clinical social work services; including crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The Social Worker is responsible for education and advocacy of patients, families and/or significant others. Participates in related department functions, such as community and program education. The Clinical Social Worker is required to work in the Emergency Department, but will be expected to support the acute medical floors as needed.

• A Master's degree from an accredited school of social work (MSW) and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners.

• Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position.

• Excellent interpersonal and communication skills.

• Must be able to work within a team environment.

• California LCSW license (must have this certification to be hired at level III).

• Proven clinical training and/or work experience in the acute care, inpatient setting.

• Must be able to work various hours and locations based on business needs.
• Employment is subject to a criminal background check and pre-employment physical.
• Minimum two (2) years of post-MSW Social Work experience is required to be hired at level II.
