Objective/Overview:

CUHCC's mission is "transforming care and education to advance health equity." This position provides direct patient care and assists in general clinic flow. This position reports to the Medical Clinic Manager and is part of the nursing team.

Essential Functions:

Patient Care/Contact:

  • Perform focused telephone and in-person assessments of people with acute health concerns to determine a safe, timely and appropriate disposition using evidence-based best practice guidelines, decision support tools, and nursing protocols. Triage may include both psychiatry and medical health patients.
  • Provide patient education, follow-up care, medications, and wound care as ordered or under protocol.
  • Complete limited medication refills per under supervision of an RN or licensed provider.
  • Respond to in-clinic emergency situations and urgent patient needs.
  • Administer care and treatment to patients as directed by providers or RNs.
  • Provide follow-ups for abnormal labs and emergency room and hospital discharges.
  • Be familiar with CUHCC's services in order to accurately refer patients within CUHCC and to appropriate outside services.
  • Consult with providers and other disciplines for specific patient needs and for overall clinic flow.
  • Coordinate follow-up care, review After Visit Summary, schedule appointments, assist with referrals, contact patients for lab results or other needs.
  • Perform procedures such as EKGs, peak flows, spirometry, dressing changes, vision and hearing screenings, ear wash, and assessments appropriate to visit type.
  • Conduct thorough and timely pre-visit planning for each encounter according to standards.
  • Assist with procedures, medical needs and patient education as they arise: EKGs, start an IV, same day referrals, lab follow up, hospital transfers, or other orders.
  • Interview patients, checks vital signs, performs assessments appropriate to patient age and visit type, obtains consents, prints out lab reports, hospital discharge summaries, and related documents.
  • Teams with other medical/nursing assistants, scribes, interpreters, laboratory and nursing staff, PSR's and others to support patient care and flow.

Other Duties:

  • Attend and participate in appropriate meetings.
  • Participate in quality improvement teams and efforts.

CUHCC reserves the right to change or add duties to this position consistent with the job classification.

A criminal background check is required for this position.

A negative TB test is required for this position.

Hourly Range: $24.24 to $28.75 USD/hour

Qualifications

All required qualifications must be documented on application materials.

Required Qualifications:

  1. Graduate of accredited LPN program and current LPN license in the state of Minnesota.
  2. Must be able to lift 30 lbs.
  3. Must be able to work in a fast-paced, rapidly changing environment that requires sitting, standing, and walking, with frequently changing priorities. Must be able to be flexible and multitask.
  4. High-level communications skills to work with patients, staff, providers, and outside agencies.
  5. Excellent customer service skills.
  6. Ability to work with patients and staff from diverse cultures, languages, and backgrounds.
  7. Capacity to work on a team and make positive contributions to the team.
  8. A negative TB test is required for this position.

Preferred Qualifications:

  1. Bilingual/Fluent in Spanish, Somali, Vietnamese, or Hmong
  2. Proficiency in utilizing an electronic medical record and basic office setting computer programs, registries or databases to monitor patient outcomes. Proficiency in electronic tools with the EMR to communicate with providers.
  3. Behavioral Health experience preferred
